Well, okay, I could be. Perhaps even should be.
But no, I'm blogging.
My silence of the last two days has been about stress and pinkeye and working the weekend, but mostly it's been about poor internet access. My little netbook, which goes back and forth to work and home, has been living at work. Today the big one gave up running our EMR software, so the little one is now hooked in to the mouse, the real keyboard, the big monitor, etc. It's all Frankenputered up and looks a little like it's on life support...but I'm taking the big one back to my folks, so, maybe there's hope. Although it's the one that has to have the power cable jury rigged juuuuust right, so who knows.
It was kind of a rough weekend. I worked. I ran some errands. I drank some coffee. I napped in spite of coffee. Oh! And I found a new apartment. Close to Shady Pines, bigger than my current one, for not much more moolah. It seems like a nice roachless townhome in a fairly quiet neighborhood. Or at least, that's what I'm hoping.
I started on the consult team today, which is a lot of apprehensive waiting. And I think we just handed off the pager. So I'm going to stop being apprehensive and go home.....

Oh, my, my, my....
Today crashed in with a vengeance.
So I'm on the Crisis unit, which is the shortened form of Adult Crisis and Stabilization, its full name. It's known for being a land of borderlines and substance abusers, at least at Baby Blue (because remember, we have seven subspecialized units. So things get sort of boiled down to their essence). It's also known for it's high drama and high turnover.
We had both today.
Now, as the third year resident, because I still have clinic responsibilities (we're supposed to be 75/25% inpatient/clinic, but it really ends up being more like 50/50), I only carry 6 patients, whereas the intern, who has no clinic, carries 9. So I was like, oh, okay, I can handle this. I had 6 on my Gero service. No big deal.
Except there's a LOT more drama, of course. And a lot more gaminess. And, oh, did I mention, I discharged 4 of those people today. Which ends up being a freakin' lot of paperwork, if nothing else.
Whew.
Well, one day down.

Blah blah blah
Today turned into a really long day. One of my coworkers is having some pretty rotten family stuff going on, so we all have sort of scrambled to take on what of his we each could. Which translated only into a couple of things for me, but one of them was going to the pre-interview dinner tonight. Which turned out to be quite lovely, actually. I met one candidate in particular with whom I definitely vibed well. She's also a knitter, and we spent a good deal of time swapping stories about leaving jobs we couldn't tolerate. It was very nice. They were fun. We laughed a lot.
But it also, as I said, extended my day by about three hours. And I'm on walk-ins again tomorrow, so I need to go to bed. But here are five random things, just to make your stopping by worthwhile. Ready? Go:
1. Apparently, high heels began as a men's fashion, related to their shoes not falling out of the stirrups when they rode. I learned this at dinner tonight. I find it hilarious, because I can't imagine most of my male cohorts doing well at all in heels (cowboy boots notwithstanding).
2. I spent a while in L&D with Eva yesterday watching her babies on the monitor (she's fine, just a little puffy). I really miss obstetrics.
3. I had a patient today who claimed to have had "just one glass" of wine yesterday, who turned out to have a BAL of 0.45 (remember, legally intoxicated is 0.08). When sober, I reminded the patient that that blood alcohol would kill me. And I'm thinkin' that was a very big glass.
4. I had another patient, a cute little old lady who's been hearing this buzzing noise, which presented like an auditory hallucination (which it may well be). And I'm asking her about it, and she's like, nope, I never hear it outside the apartment, but it's almost constant there. And I was like, has anyone else listened for it? And her boyfriend says, "NO. I'VE NEVER HEARD IT." And I giggled a little bit - inwardly, of course - and thought, turn your hearing aids up, grampa.
5. I really like this picture, which I stole off of Twitter. I was listed as, "Not a hot dog, but a warm dog."

It's 6:40pm...
...and I'm thinking of going to bed.
I had candy corn for dinner. I mean, like, a handful, not a bag, but it's still not so good. And my stomach hurts now.
I'm so completely wiped out.
Got my flu shot today. Took both days' worth of our inservice exam. Got home and had a very fucking obnoxious voicemail from one of my cohorts. So I'm passive-aggressively ignoring what they asked me to do because a) it's ridiculous and 2) it's all I can do to stay awake.
I had such plans. Winding yarn, starting to pack, reading, dying my hair, working on a discharge summary or two. Y'all, I seriously can't keep my eyes open...
The point of all this: Wordless Wednesday has been postponed to Wordless Thursday.
Yeah, so clearly not wordless today....

Lateness and brevity
It is after midnight, here. I had a wicked long day covering the ECT service for Fang. I left the hospital at 8pm, and then I went and grabbed some dinner with Sparrow and Ruthie. And then I drove home, and discovered a cool little package on my doorstep, which I of course had to open and shoot multiple pictures of, and will tell you about on Sunday.

Sit. Stay. (Maggie Speaks)
Hi everybody! Hi! Hi! It's Maggie. Kate...well...Kate seems to have passed out in the middle of the piles of stuff on the big bed. So I thought I'd write the blog for her. She's snoring a little, so I'm pretty sure she's not dead. I keep trying to tell her, the longer she stays away from here, the more tired she is when she gets home, and so she should really stop that. Frankly, I don't know why she leaves here, ever. Unless maybe she goes to the Daycare. That's a good place to go. I always need a nap when I get home from there, too, even when I stay overnight.
She doesn't seem to be going to my Daycare, though. Last time she was gone all night, Sparrow and I went for a big long walk. It smelled really interesting, and at the end we found Kate!! It was so exciting! Tyler was there, too, and Sparrow, of course, and these two other guys who rubbed my head, but neither of them smelled like they have packs at home. The Daycare they were at was really big, and smelled kind of funny. And there weren't other dogs there. And they didn't seem to have a pool. And everyone seemed tired, and the little screaming black box that Kate clips on her pants kept screaming. I'm not sure why she goes there so much.
Oh! Did you hear that? Is that a squirrel? Or maybe someone's invading. Or maybe they've got food. I'd better go check...
Today Kate smelled really good when she got home. She was all salty, too. I think she said she was with Jim. I don't know who this Jim guy is, but she's spending a lot of time with him these days. And she always comes home all damp and salty. Apparently he knows Gomer. I think maybe they're part of the same pack. So she went to this Jim today, and then came home and ate some of those salty triangles with the red stuff on them and she didn't drop nearly enough of them. They're better with some of the red stuff on them, but Kate never gives me much of that.
Oh! Oh! Guess what! Mom and Dad are here! They came this afternoon in the big loud white thing. Dad seems to have more fur on his face now. Maybe he hasn't been to the groomer recently. Mom brought cookies. Kate said I couldn't have any, but when they went for dinner, they came back, and Mom brought me a Jumbone! She let Kate give it to me, because Mom doesn't like it when I get too close. Kate said she had a dog in her pack when she was a puppy who was mean to her. I think that's sad. I like Mom, but I try to let her have her space so she won't be sad. Dad rubbed my head, though! And my tummy. I miss them. They go away for much too long.
They kept bringing in all these boxes. I'm not sure what that's about. They don't seem to have anything in them. I think it means we're going somewhere. I really hope Kate takes me with. I always get worried, when this happens. Sometimes, she just packs up that one black box with wheels, and then she's gone for too long. When she starts packing boxes like this, though, we usually wind up living in a whole different den (she calls it a "house." Whatever). She's never left me behind before, and we've been in a few different houses at this point. But I worry. I worry that she might leave and not take me with. I worry that she might leave and never come back!! See, I think the best option is just for her to never, ever leave. Or, you know, take me with her, always. Because packs need to stay together.
Okay, well, I've been awake for, gosh, like an hour now, so I'd better take a nap. I think tomorrow's going to be busy. Kate keeps saying things like "packing extravaganza" and "how in the hell are we going to get this whole place packed in two days?" Mom seems less worried. But, yep, better get to sleep. Thanks for coming by! Next time bring food, okay? Maybe Kate will let me write the blog again soon. It's kind of hard to type without thumbs, but, I think I was a good girl.

Fun with pictures
Call it Wordless Tuesday. Except with words. Lots of 'em.
So Sparrow and I are on together tonight. Which, so very rocks.
(PS - did I mention that Sparrow had a birthday last week? I think I forgot. She had her first 29th birthday this past week. We went out for "Asian Fusion" - if several days later - and it was yummy. The company was impeccable, of course, as well...)
(Ruthie's birthday is today, also. I made an impromptu little banner on our door using Post-It notes.)
(But I digress.)
So Sparrow and I are here. And we had a lot of funny, funny stuff come across our paths last night. So we decided to take pictures (you'll notice that all the patient identifiers are conveniently missing...).
Shortly after I came on, a direct admission came from one of the community hospitals. I went down, signed her in, talked to her a bit, and then came back upstairs and looked through the thick envelope of material that the outside hospital sent with her.
I found about twenty pages of MARs (medication administration records), which was ridiculous because this woman is on exactly ONE medication. And of course, they sent a big stack of nursing notes.
Notably missing? A discharge summary. Or, say, a psych consult note. But in case you were worried, we got her post-endoscopy nursing notes, too.
Absolutely essential to good psychiatric care.
They also...seriously, I almost fell off my chair when I saw this. This is not a diagnosis.
The word is hypokalemia. I mean, really. They are just makin' shit up at this point.
You know you're in good hands when your doctors start making up diagnoses just for you. That's a sure sign that they really know what they're doing.
Also waiting for me when I came in was this hand-off note:
Oh, but, be sure to notice the patient's age.
It's the the Playskool Trenchcoat Mafia out there. They're just starting younger and younger these days. Sparrow tells me she got a report from a parent the other day that some three year old girl was "pole dancing". Really? A three year old taking off her clothes. Astonishing. That's highly unusual. Toddlers almost never like to run around naked.
And furthermore, pole dancing is not an innate talent, unless you're a surgeon (inside joke. Sorry...). So if you find your little one working the pole at Gymboree...you've got ask yourself where she could possibly have learned that behavior.
I'm just sayin'.

Cuddly
So I'm trying to stay up late. Because I'm on night float next week (LAST WEEK OF NIGHT FLOAT EVER. EVER.) and am endeavoring to get my sleep schedule flipped. Not that it's going to work, but...well, whatever. Did I mention this is my last week of NF?
I had a long day. I had patients at 9, 10, 11, 12, 1, 2:30, 3:30, and 4:30, and, I had my yearly review with my program director at 2, plus a supposed lecture at 8 (the lecturer got stuck in traffic, so, I got about 30 minutes of solid administrative time out of the deal instead. Ahhh). I did have dinner with Ruthie, though, after all of this therapizing, which was very nice. We went to an Italian place in town and had excellent food and this really, really good pie. Mmmm.
And somewhere towards the end of that evening we had a whole conversation about, if people in our residency were animals, what would they be? This was spurred on by a discussion of people's stupid nicknames on the blog (Fang, Gomer, May and June, Mike and Ike, Benny, Martha, Faye, Rene, Tony and Cleo, etc, etc, etc, but most notably leading to the former conversation were, of course, Sparrow and PenguinShrink).
Somehow...Ruthie decides that I'm a Care Bear.

Cracks. Me. Up.
I tried to tell her they weren't real. Her answer? "Please. Of course they are."
She decided I'd be a pink Care Bear, with cute little bear ears, who was really very cuddly. I've decided this makes me Love-a-Lot Bear (who was one of my favorites when I was six. I definitely had the little jointed action figure).
I could be Love-a-Lot Bear.

Call-ful thinking
Dear Triangle Area Crazy People:
Please stop coming into our ER tonight. We have no more psych beds, and the doctors are tired and want to go to sleep. I've heard, though, that if you go to that other hospital (you know, the one down the street. They're a slightly darker shade of blue than we are) they have wide screen TVs in all the patient rooms and super comfy mattresses and gourmet food and they sing you to sleep at night.
No, really.
Think about it.
Why would anyone want to come here? Last year, one of Ruthie's patients kept finding lice in her bed. I mean, no one else could find the lice in her bed, and they kind of went away when we finally got her meds right, but really, why risk it?

Things that seem hysterically funny when you've been awake all night.
Seriously. I had to turn the sound down to almost nothing, because otherwise I was laughing myself into hyperventilating fits.
Ahhh, those were the days, back before Doogie was gay. When Vinnie was annoying but kind of adorable, when gaggles of big-haired girls all secretly hoped Doogie would realize what a bitch Wanda was and dump her ass, and when Mrs. Howser was the original MILF to legions of boys in Hammer Pants.
Then again, I do still love me some NPH...
Tonight was the first night of Night Float, and it wasn't bad. I got all my dictations finished, all (er, well, most) of my filing done, and even saw a few patients. Some little old lady up on the floor broke her hip, and is off to the OR this morning, and she and the nurses are now in love with me and my liberal use of IV dilaudid for sweet little old ladies who fall and end up with impacted femoral neck fractures.
I'm a little more delirious than usual, though. I was supposed to see my shrink this morning, but then I called her to reschedule when I realized the last two sentences I'd written in the H&P I'd started at 6:30 on the patient I saw at 4:45 (it was now 7:30 at this point, if that tells you anything) made absolutely no sense. But I've since had caffeine (I couldn't help myself) and am a little more lucid, and, well, we'll see....
Tomorrow night (er, tonight, I guess, technically), I'm hoping for an equally calm night. Tyler's birthday is tomorrow (no, like, actual tomorrow, so, Tuesday, really), and Sparrow is on with me overnight, so she's bringing a cake to pull out at midnight. Plus, I really need to get some knitting done. And now that I've said that and angered the NF gods, well, I have this bad feeling we're going to admit the whole county...
Ooh. Wait. I live in this county. If I get admitted, maybe I could get some sleep....

I was supposed to see my shrink this morning, but then I called her to reschedule when I realized the last two sentences I'd written in the H&P I'd started at 6:30 on the patient I saw at 4:45 (it was now 7:30 at this point, if that tells you anything) made absolutely no sense. But I've since had caffeine (I couldn't help myself) and am a little more lucid, and, well, we'll see....
Tomorrow night (er, tonight, I guess, technically), I'm hoping for an equally calm night. Tyler's birthday is tomorrow (no, like, actual tomorrow, so, Tuesday, really), and Sparrow is on with me overnight, so she's bringing a cake to pull out at midnight. Plus, I really need to get some knitting done. And now that I've said that and angered the NF gods, well, I have this bad feeling we're going to admit the whole county...
Ooh. Wait. I live in this county. If I get admitted, maybe I could get some sleep....
